Eight: cube, cuboid, cylinder, cone, triangular prism, square pyramid, pentagonal pyramid and hexagonal prism. They fold up one at a time and can stand side by side, so the differences become visible.
Can the shapes be folded flat again?

Yes, that's the idea. Each shape folds up from its page and lies flat again so the book can close. That's also what makes the book usable more than once. The folding is the exercise.
